Job Summary:

As a Home Care Aide with Kaiser Permanente, you will provide personalized care and support services to patients in their homes. You will work collaboratively with our hospice/home health team to deliver high-quality care that meets the unique needs of each patient. You will report directly to the RN case manager or PT/OT/ST, ensuring seamless communication and coordination of care.

Key Responsibilities:
  • Assist patients with personal care activities, such as bathing, skin care, oral hygiene, and toileting, under the supervision of the RN or PT/OT/ST.
  • Support patients in carrying out their prescribed therapeutic regimen, including active/assistive range of motion and positioning, safe transfer techniques, and ambulation.
  • Provide household chore services essential to healthcare at home, including meal preparation, linen change, personal laundry, and dishwashing.
  • Monitor and document patients' condition, including vital signs and care or service furnished, and report any changes to the RN or PT/OT/ST.
  • Recognize and respond to emergency situations as directed, ensuring timely and effective care.
  • Apply basic infection control procedures to maintain a clean, safe, and healthy environment for patients.
  • Document patient information accurately and timely in the clinical record, adhering to hospice policies.
  • Reinforce education provided by the Case Manager to patients and caregivers, promoting a collaborative approach to care.
  • Provide emotional support and compassionate care to patients and their families, demonstrating active listening skills and empathy.
  • Review medical records to validate assignments and HCA orders, ensuring accuracy and completeness.
  • Participate in performance improvement activities, contributing to the development and implementation of departmental workplans.
  • Prepare weekly schedules of patient visits and maintain open communication with supervisors and intake coordinators.
  • Rotate weekends and holidays with other HCAs as assigned, ensuring seamless coverage and care.
  • Participate in interdisciplinary team conferences, sharing knowledge and expertise to enhance patient care.
  • Attend continuing education and in-service training programs to stay current on patient care techniques and needs, including competency evaluations.
  • Maintain a current CQA driver record information sheet, ensuring a safe and reliable driving record.
Requirements:
  • Minimum of one year of experience as a Home Care Aide, with successful completion of a training program in the area of specialty, if applicable.
  • Completion of a nurse aide program or equivalent related experience.
  • Driver's License (Hawaii) and Basic Life Support from the American Heart Association.
Preferred Qualifications:
  • Experience in and ability to support postmortem care.
  • Exhibits compassion in helping to care for patients and families who may be experiencing grief and loss.
  • Great interpersonal, communication, and collaboration abilities, as well as an ability to provide compassionate end-of-life care.
  • Exceptional ability to manage stress and emotional challenges.
  • Competency in electronic patient and hospice management systems, such as Epic.
