Job Overview

Hamilton Families is seeking a dedicated Case Manager, Stability, to join our Housing Solutions team. This role plays a vital part in our mission to end family homelessness in the San Francisco Bay Area.

The Case Manager, Stability, will provide home-based support to approximately 20 families, including goal setting, housing advocacy, and connecting them to resources for budgeting, employment, legal support, and more.

Key Responsibilities:

  • Assess participants' needs and challenges, such as employment status or mental health, and connect them to appropriate resources
  • Assist families in setting and reaching housing goals, manage necessary documentation, and address related concerns
  • Check in with participants who have completed the program to ensure continued progress
  • Foster self-sufficiency and maintain professional boundaries using various service models
  • Collaborate with internal teams and external partners to address housing issues and support participants
  • Maintain accurate records and adhere to confidentiality documentation standards

Requirements:

  • 1+ years of experience in social work, psychology, human services setting, or related field
  • A background in working with nonprofit organizations, social services organizations, or community-based programs
  • Familiarity with Bay Area resources for low-income families
  • Bilingual abilities, especially in Spanish
  • Highly proficient in Microsoft Office Suite, Salesforce, and other software

Salary and Benefits:

$28.00/hour, bilingual premium (Spanish): $1.50 extra per hour, full-time, non-exempt position, union representation, Monday – Friday, 9:00am – 5:00pm, location: 2567 Mission St with often travel to various housing locations across the Bay Area
