Job Summary

Hamilton Families is seeking a skilled Case Manager - Stability to join our Housing Services Department. The successful candidate will provide case management services to families participating in our rental subsidy program, supporting them to achieve housing stability and meet their goals.

Key Responsibilities:

  • Provide case management services to a caseload of approximately 20 families across 6 counties.
  • Assess participants for strengths and barriers, and refer them to appropriate resources.
  • Assist participants to establish and meet housing goals, and identify steps towards achieving those goals.
  • Obtain necessary documentation from participants, including income verification and proof of rental payment.
  • Complete follow-up calls with participants after program completion.
  • Promote values of self-sufficiency and empowerment throughout work with participants.
  • Reinforce and follow appropriate boundaries with participants.
  • Coordinate and collaborate effectively with internal and external service providers and community partners to assist participants to reach their goals.

Requirements:

  • Bilingual Spanish preferred but not required.
  • A minimum of 1 year of relevant professional experience OR a bachelor's degree in social work, psychology, or a related field from an accredited college or university.
  • Able and willing to work with diverse staff and participants.
  • Familiarity with, or ability to quickly learn, technology platforms including case management databases and customer service platforms.
  • Experience working with extremely low-income families, youth, survivors of domestic violence, populations impacted by substance abuse, mental illness, HIV/AIDS and other diverse communities.
