RN Hemodialysis Care Coordinator

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hemodialysis Care Coordinator. In this role, you will be responsible for coordinating the delivery of high-quality patient care and collaborating with the healthcare team to achieve optimal patient outcomes.
Your key responsibilities will include assessing patient needs, developing individualized care plans, and implementing evidence-based interventions. You will also be responsible for monitoring patient progress, making adjustments to treatment plans as needed, and communicating with physicians and other healthcare professionals.
To be successful in this role, you will need to have a strong foundation in nursing principles and practices, as well as excellent communication and interpersonal skills. A minimum of 9 months of experience as a Registered Nurse is preferred, with at least 3 months of experience in chronic/acute hemodialysis. Graduation from an accredited School of Nursing and current licensure are required. CPR BLS Certification is also necessary.
Salary Estimate: $65,000 - $80,000 per year, based on location and experience.
