About the Role:
We are seeking a skilled Registered Nurse to join our Home Health team in Melbourne, Florida. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.
Responsibilities:
- Conduct comprehensive assessments of patients to identify their unique needs and develop effective care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure seamless care coordination.
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
Requirements:
- Current RN licensure in the state of Florida.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
