Patient Care Coordinator

We are seeking a highly skilled Registered Nurse-Labor to join our team at Northwell Health. As a valued member of our healthcare team, you will play a critical role in delivering high-quality patient care and contributing to the overall success of our organization.
Key Duties and Responsibilities- Conduct patient assessments, develop and implement individualized plans of care, and coordinate with physicians and other healthcare professionals to ensure seamless care delivery.
- Collaborate with interdisciplinary teams to promote safe and effective care practices, including medication management and wound care.
- Participate in quality improvement initiatives, contribute to the development of policies and procedures, and maintain accurate documentation of patient care activities and outcomes.
- Develop and lead educational programs for patients and their families on health promotion, disease prevention, and management of chronic conditions.
- Graduate from an accredited School of Nursing; Bachelor's Degree in Nursing preferred.
- Current License to practice as a Registered Professional Nurse in New York State required; specialized certifications as needed.
- At least 1-2 years of experience in a healthcare setting, with a strong track record of delivering exceptional patient care.
The salary range for this position is estimated at $64,835 - $92,625 per year, based on a 30-hour workweek. We offer a comprehensive benefits package, including health insurance, retirement savings plan, paid time off, and opportunities for professional growth and development.
