Job Title: Mental Health Clinician Case Manager

We are seeking a highly skilled Mental Health Clinician Case Manager to join our team at The University of Vermont Medical Center.

About the Role:

The Mental Health Clinician Case Manager will work as part of a multi-disciplinary team to provide evidence-based outpatient services to individuals with primary substance use disorders and co-occurring mental health conditions.

Key Responsibilities:
  • Evaluate new patients and diagnose substance use and co-occurring mental health disorders in accordance with the DSM and ICD.
  • Assess for appropriate level of care using ASAM criteria.
  • Provide direct patient care, including individual, family, and group psychotherapy.
  • Consult with primary care and other referring providers in the community.
  • Present cases at weekly clinical rounds.
  • Coordinate clinical services with other internal and external providers.
  • Formulate the plan of care, update the plan as needed, and coordinate treatment for the patient while in the program.
  • Continued care and discharge planning, including scheduling appointments with addiction, mental health, medical, and peer support providers.
Requirements:
  • Master's degree in a behavioral health or related field required.
  • Active Drug and Alcohol Counselor License (LADC) in the State of Vermont required.
  • Active additional licensure as either a clinical social worker or clinical mental health counselor in the State of Vermont strongly preferred.
