Medical Social Worker, Home Health Full Time

We are seeking a skilled Medical Social Worker to join our team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in providing comprehensive care to our patients in the comfort of their own homes.
Key Responsibilities- Evaluate and develop personalized plans of care to meet the emotional and social needs of patients
- Assess patients' social and emotional states, and provide guidance on available community resources
- Collaborate with healthcare teams to ensure seamless care coordination
- Supervise and evaluate the performance of Social Work Assistants to ensure high-quality care
- Document patient progress and maintain accurate records
- Masters or doctoral degree in Social Work from a Council on Social Work Education-accredited program
- Current licensure as a Social Worker in the state of practice
- Minimum one year of experience as a Social Worker in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
