**About the Role:**
We are seeking a compassionate and skilled Home Health Aide to join our team at Essentia Health. As a Home Health Aide, you will play a vital role in providing personalized care and support to our residents, helping them with their daily needs and promoting their overall well-being.

**Responsibilities:**

* Provide high-quality patient care and support, including assistance with daily living activities, medication management, and mobility
* Work collaboratively with our interdisciplinary team to develop and implement individualized care plans
* Maintain accurate and detailed records of patient care and progress
* Communicate effectively with patients, families, and healthcare team members

**Requirements:**

* 1 year of experience as a Home Health Aide or Nursing Assistant Registered
* Active certification on the Wisconsin State Registry with the ability to obtain certification on the Minnesota Registry within 3 months of hire date
* Current Basic Cardiac Life Support (BCLS) certification
* Possession of a valid motor vehicle operator's license and current motor vehicle insurance
