We are seeking a highly skilled and experienced Facility Improvement Analyst to join our team at Heluna Health. As a key member of our Housing and Support Services team, you will play a critical role in ensuring the quality of services and accommodations for our clients in Residential Care Facilities (RCFs) and Assisted Living Facilities (ARFs).
Key Responsibilities- Conduct Quarterly Facility Visits: Conduct regular visits to RCFs and ARFs to assess the safety and quality of facility services, identify areas of concern, and develop improvement plans.
- Assess and Grade Facilities: Utilize a standardized tool to assess and grade facilities, providing recommendations for improvement.
- Develop Improvement Plans: Collaborate with facility operators to develop and implement improvement plans, providing technical assistance and training as needed.
- Communicate with Stakeholders: Communicate with the Referral and Placement Team, Los Angeles Department of Mental Health, and other stakeholders to ensure seamless coordination and communication.
- Maintain Facility Lists: Maintain up-to-date master facility lists in our database, ensuring accurate and timely information.
- Coordinate Learning Communities: Convene quarterly learning communities with facility operators to share best practices and address challenges.
- Oversee Contracts and Agreements: Oversee all ERC contracts and agreements, working with the data team to finalize reporting to funders and internal stakeholders.
- Experience: Three years of experience in a staff capacity analyzing and making recommendations for problem-solving, or equivalent experience in health care or community/social service operations.
- Certifications/Licenses/Clearances: Valid California Class C Driver License or ability to utilize alternative transportation, and successful clearing through the Live Scan and Health Clearance process.
- Skills and Knowledge: Homeless health care experience, experience with delivering clinical care services in RCFs and ARFs, familiarity with complex care management services for high-need/high-cost MediCal beneficiaries, and ability to lead teams in performance improvement activities.
