Job Description:
Kaiser Permanente is seeking a licensed practical nurse (LPN) to join our ambulatory care team in Portland, Oregon. The successful candidate will work closely with clinicians and registered nurses to deliver high-quality patient care and support to patients with routine or self-limiting medical conditions.

Responsibilities:
* Assess and manage patients with routine or self-limiting medical conditions as directed by RNs or clinicians.
* Perform clinical tasks and procedures under the direction of a clinician or registered nurse.
* Assist with health maintenance and care gap identification and address with patients and alert clinicians as necessary.

Requirements:
* Current LPN licensure in Washington and Oregon.
* Graduation from an accredited school of nursing.
* Basic Life Support certification.
