POSITION OVERVIEW
The licensed practical nurse is responsible for executing tasks and duties within the scope of patient assessment, health education, health counseling, and providing supportive and restorative care under the supervision of a licensed healthcare professional.

KEY ATTRIBUTES

Valid, unrestricted New York State practical nursing license

Completion of an accredited practical nursing program

Demonstrates understanding of medications, their administration, and potential side effects.
KEY RESPONSIBILITIES
Gather patient information to assist the licensed healthcare provider in delivering comprehensive patient care.
Collect vital signs, allergies, current medications, medical history, social history, family history, and chief complaints, documenting them accurately in the electronic medical record.
Identify issues within the practice and communicate them to management. Maintain a clean and organized workspace to ensure a safe environment for patient care. Support all initiatives of the healthcare team and promote exceptional customer service. Collaborate with the PPM team to address clinical concerns and identify areas for improvement.Engage in special projects as assigned.
Work with the PPM team to resolve daily operational challenges and ensure compliance with quality patient care standards.
Integrate the organization's mission and core values into daily activities. Collaborate with CPH and PPM Departments as necessary.Provide and document direct patient and family-centered care in accordance with nursing standards.
Prepare patient charts to ensure all necessary information is available prior to patient consultations.
Deliver routine nursing care and treatment to patients under the supervision of a licensed healthcare provider, collect specimens, perform medication reconciliation and administration, and assist providers with basic procedures.
Adhere to administrative and nursing service policies and procedures.Effectively manage medical and clerical supplies to control practice expenses. Accurately document all treatments performed during patient interactions and assign appropriate CPT codes as required.
Collaborate professionally with all members of the healthcare team in delivering patient care. Work with the PPM team to meet all established standards required under DNV accreditation. Maintain knowledge of state and federal healthcare regulations as applicable. Comply with CPH and PPM policies, procedures, guidelines, and protocols.
PHYSICAL DEMANDS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing, or squatting.
COMPENSATION RANGE:
$ $36.47
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
