POSITION OVERVIEW

The licensed practical nurse is responsible for executing tasks and duties within the scope of casefinding, health education, health counseling, and providing supportive and restorative care under the supervision of a licensed healthcare professional.

KEY ATTRIBUTES


Mandatory:

Valid, unrestricted New York State practical nursing license

Mandatory:

Completion of an accredited practical nursing program

BLS certification within three months of employment

Demonstrates understanding of medications, their administration, and potential side effects.

KEY RESPONSIBILITIES
Gather patient information to assist the licensed healthcare provider in delivering comprehensive patient care.

Record vital signs, allergies, current medications, medical history, social history, family history, and primary concerns, ensuring accurate documentation in the electronic medical record.

Identify operational issues within the practice and communicate them to management. Maintain a clean and organized work environment to ensure patient safety. Support all initiatives of the healthcare team and promote exceptional customer service. Collaborate with the PPM team to address clinical challenges and identify areas for improvement.
Engage in special projects as assigned.

Work alongside the PPM team to resolve daily operational challenges and ensure compliance with quality patient care standards.

Incorporate CPH mission and core values into daily operations. Collaborate with CPH and PPM Departments as necessary.
Provide and document direct patient and family-centered care in accordance with nursing standards.

Prepare patient charts to ensure all necessary information is available prior to patient consultations.

Deliver routine nursing care and treatment to patients under the guidance of a licensed healthcare provider, collect specimens, perform medication reconciliation and administration, and assist providers with basic procedures.

Adhere to administrative and nursing service policies and procedures.
Utilize medical and clerical supplies efficiently to help manage practice expenses effectively. Document all treatments performed during patient interactions and assign appropriate CPT codes as required.
Engage professionally with all members of the healthcare team in delivering patient care. Collaborate with the PPM team to meet all established accreditation standards set by DNV. Stay informed of state and federal healthcare regulations as applicable. Comply with CPH and PPM policies, procedures, guidelines, and protocols.

PHYSICAL DEMANDS:

M - Medium Work - Involves exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects; Requires frequent walking, standing, or squatting.


COMPENSATION RANGE:
$ $36.47
The stated base pay range is a good faith representation of potential base pay for successful candidates. It may be adjusted in the future. Compensation is influenced by factors such as experience, relevant qualifications, specialty, internal equity, location, and contractual agreements.
Minimum Salary: 24.89
Maximum Salary: 36.47
