Key Responsibilities:
The RN Case Manager is tasked with conducting comprehensive physical and psychosocial evaluations for patients in the final stages of life. This role involves developing and executing a nursing care plan and making informed recommendations based on the individual needs of patients. The RN will also supervise, train, and assess home health aides, homemakers, and personal care assistants within patients' residences. Additionally, the RN will request written directives from physicians for any necessary modifications to the patient care plan.

The position requires participation in an On Call rotation 2-3 times per month and availability for 2-3 holidays each year.

Qualifications:
- Associate's Degree or Diploma from an accredited Nursing program; BSN preferred

- Minimum of two years of experience in medical-surgical, community health, and/or oncology nursing preferred

- Proficiency in Microsoft Word and Excel preferred

- Valid New York State RN licensure and a current NYS driver's license required

- Experience in a collaborative interdisciplinary team environment

- Strong communication, listening, and teaching abilities

- Competence in accurately inputting clinical data
