Discovery Village at Boynton Beach is seeking a compassionate and dedicated Memory Care Coordinator to join our team.
Job Summary:The Memory Care Coordinator will be responsible for designing, scheduling, and facilitating a comprehensive Memory Care program that incorporates Life Skills and dementia-appropriate activities. This role will partner with the Celebrations Director to ensure a variety of activities are available throughout the day and evening, and that Care Managers are actively involved and engaged with activities.
Responsibilities:- Design and implement a Memory Care program that meets the needs of residents with Alzheimer's disease or related dementia.
- Partner with the Celebrations Director to ensure a variety of activities are available throughout the day and evening.
- Support the clinical team in ongoing in-service education to the Memory Care staff on disease processes and population being served.
- Refer clinical issues to the Director of Health and Wellness to ensure residents' clinical needs are addressed.
- Communicate with Memory Care team regarding resident preferences and needs prior to move-in.
- Provide a point of contact for the Director of Health and Wellness regarding Service Plan and Daily Assignment Sheet development.
- Act as the community champion in Memory Care programming and activities.
- Collaborate with dining services to ensure residents' individualized dining needs and preferences are met.
- Report any resident changes in condition to DHW and ED.
- Ensure pertinent information from the Daily Log is communicated to DHW/ED and Team as appropriate.
- Collaborate with leadership team to develop specific programming and approach mechanisms tailored to meet residents' needs.
- Serve as the family point of contact for residents of the Memory Care Program.
- Collaborate with leadership team to maintain operations of the program within state regulations.
- One (1) year of experience in a healthcare environment.
