Job Description

We are seeking an experienced Home Health Physical Therapist to join our fast-growing team. In this role, you will work closely with our team to help patients achieve their highest level of physical function. Your responsibilities will include:

  • Developing and implementing treatment programs consistent with the physician's plan of treatment.
  • Conferencing with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program.
  • Providing written instructions and care plans for Physical Therapy Assistants and Home Health Aide staff.
  • Accurately documenting patients' care observations, interventions, and evaluations.

Requirements

To be successful in this role, you will need to have:

  • An advanced degree of Doctor of Physical Therapy (DPT) from an institution accredited by the American Physical Therapy Association (APTA).
  • A current and unrestricted license to practice Physical Therapy in the State of California.
  • Current CPR/BLS certification.
  • Possess up-to-date knowledge of treatment practices and physical therapy programs.
  • A minimum of 1 year of recent Physical Therapy experience in an institutional setting.
  • A minimum of 1 year of recent Physical Therapy experience with a home health agency.
