About the Role

We are seeking an experienced Physical Therapist to join our team as a Home Health Therapist. The successful candidate will be responsible for providing rehabilitative care and skilled nursing to clients in their homes, promoting independence and improving quality of life.

Responsibilities
  • Conduct home visits to assess client needs and develop personalized care plans
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to improve client mobility and function
  • Educate clients and family members on rehabilitation techniques and strategies to promote independent living
  • Continuously assess and revise care plans to ensure optimal outcomes
Requirements
  • Current PA Physical Therapist license
  • Minimum one year of recent work experience as a Physical Therapist
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently and manage time effectively
  • Strong interpersonal, organizational, and problem-solving skills
