Full-Time Outpatient Physical Therapist - Home Health in Senior Living Communities

BAYADA Home Health Care is currently seeking an experienced Physical Therapist for a Full-Time opportunity performing outpatient visits in Senior Living Communities. This Physical Therapist will treat geriatric clients who are residents of Assisted Living and Independent Living Communities in Centerville, Hyannis and Yarmouth You will be instrumental in optimizing function and independence for older adults through proactive evidence-based interventions.

You will also participate in client screens and wellness events.

One year prior clinical experience as a licensed Physical Therapist is required. As an outpatient Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides outpatient services to clients affording them the opportunity to age in place.

Responsibilities for an Outpatient Physical Therapist include:

  1. Make outpatient visits to clients in designated senior living communities.
  2. Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
  3. Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
  4. Continually assess and revise the Physical Therapy - PT plan of care, and participate in the multidisciplinary care plan meetings including community staff, as appropriate.
  5. Educate and instruct clients, family members, or community staff, in therapy services and activities necessary to promote the client's health, safety, and independent living.
  6. Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.

Qualifications for a PT - Physical Therapist include:

  1. A current Massachusetts Physical Therapist license.
  2. A minimum of one year of recent work experience as a Physical Therapist.
  3. Outpatient experience a plus but not required.
