About the Role

We are seeking a highly motivated and compassionate Case Manager to provide support and guidance to our clients as they work towards achieving self-sufficiency and permanent housing.

Key Responsibilities
  • Develop and implement individualized plans to help clients achieve their goals
  • Provide on-site services and support, including direct service to clients in areas such as money management, daily living skills, and work readiness skills
  • Transport clients to appointments and activities as needed
  • Utilize an electronic health record system to document client progress
  • Participate in ongoing training and professional development to enhance skills and knowledge
Requirements
  • High school degree required, Associates or Bachelors degree preferred
  • Ability to pass criminal/registry background checks/drug screen
  • A valid driver's license, no more than 5 points or DUI in last 3 years
  • Ability to pass and maintain required Agency and program-specific trainings
