Key Responsibilities:

  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments of patients' needs and provide education on disease management and self-care.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Manage and coordinate home health services, including medication management and equipment needs.

Requirements:

  • Current and unencumbered RN license in the state of employment.
  • Bachelor's degree in Nursing from an accredited program, preferred.
  • One to two years of recent acute care experience in an institutional setting.
  • One year of home health care experience, preferred.
  • Management experience not required, but direct supervisory experience of LVNs and NAs, preferred.
  • Possess and maintain valid CPR certification.
  • Working knowledge of OASIS documentation requirements, preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Access to reliable transportation.
