Key Responsibilities:
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Conduct thorough assessments of patients' needs and provide education on disease management and self-care.
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Manage and coordinate home health services, including medication management and equipment needs.
Requirements:
- Current and unencumbered RN license in the state of employment.
- Bachelor's degree in Nursing from an accredited program, preferred.
- One to two years of recent acute care experience in an institutional setting.
- One year of home health care experience, preferred.
- Management experience not required, but direct supervisory experience of LVNs and NAs, preferred.
- Possess and maintain valid CPR certification.
- Working knowledge of OASIS documentation requirements, preferred.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Access to reliable transportation.
