About the Role:
We are seeking a highly motivated and compassionate Nurse Care Coordinator to join our interdisciplinary team. In this critical role, you will work closely with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care, ensuring seamless transitions and optimal outcomes.
Key Responsibilities:
- Coordinate patient care across the continuum, from hospital to home

Requirements:
