Job Title: Social Worker - Delta/Montrose

About the Role:
  • Work 20 hours per week in each location, offering flexible scheduling and a mobile office
  • Part of an interdisciplinary care team, collaborating with social workers, chaplains, nurses, physicians, and certified nursing assistants
  • Advocate for patients and families, providing education and resources to support their needs
  • Work with a robust team focused on providing quality care to patients in the community
Requirements:
  • MSW with Colorado Social Work licensure preferred, BSW required
  • Minimum of one year supervised experience in the healthcare field or during a supervised internship
  • Excellent interpersonal and communication skills
  • Ability to commute between Delta and Montrose, CO
