Job Summary:

M Health Fairview is seeking a Visit Facilitator to support in-person clinic visits and guide patients through the care experience.

Responsibilities:

  • Support patients through the care experience, capturing pertinent medical information and detailing it into the medical record.
  • Ensure exam rooms are stocked, equipment and supplies are available, and assist with tasks that support the effectiveness and efficiency of the team.
  • Prepare patients for appointments, including communicating prior to their scheduled appointment to ensure medical records are collected and available for the visit.
  • Assist in the rooming process by collecting and detailing pertinent information, including reason for visit, medication review, vitals collection, and health questionnaires.
  • Support the patient connection to the provider for the visit and serve as a resource ensuring a positive patient/provider experience.
  • Complete visit wrap-up and provide after-visit summaries to patients, including assisting with scheduling and coordinating follow-up appointments.
  • Communicate efficiently with patients, providers, and care team, demonstrating comfort in person and with virtual technologies.

Qualifications:

  • At least 1 year of customer service experience.
  • Proficient with technology, including computer hardware and programs, and smart devices such as phones or tablets.
  • Preferred qualifications include experience in customer service, medical terminology, experience with Epic and/or experience working in a healthcare setting.
