Job Summary

We are seeking a skilled Registered Nurse to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating patient care teams, providing and documenting patient care, and managing health care and education for patients and their families.

Key Responsibilities
  • Coordinate patient care teams to provide and document patient care in accordance with applicable standards of practice.
  • Apply the nursing process to all areas of practice, prioritizing patient care needs and providing care accordingly.
  • Manage and provide health care and education to patients and their families during hospitalization.
  • Assess, implement, coordinate, delegate, and evaluate nursing care of patients.
  • Perform specialized nursing care procedures as outlined in department/unit-specific competency profiles.
  • Coordinate patient encounters, admissions, transfers, and discharges.
  • Provide leadership to and development of patient care teams.
  • Identify opportunities for improvement in patient care and organizational processes.
  • Participate in performance improvement activities and medical and nursing research activities.
Requirements

To be successful in this role, you will need to have a strong background in nursing, excellent communication and leadership skills, and the ability to work effectively in a fast-paced environment.
