Care Coordinator Nurse

We are seeking a skilled Registered Nurse Care Coordinator to join our team at Nationwide Children's Hospital. As a key member of our interdisciplinary team, you will play a vital role in ensuring quality patient care and coordination of services.
Responsibilities:- Assess patient and family needs, developing individualized plans of care in collaboration with the healthcare team.
- Coordinate transitions between settings, caregivers, and providers to ensure seamless care.
- Facilitate communication and collaboration among the healthcare team to support complex patient needs.
- Participate in ongoing education and training to stay up-to-date on information technology requirements and best practices.
- Current licensure as a registered nurse in the state of Ohio.
- Valid Ohio driver's license and proof of auto insurance.
- Pediatric certification, Advanced-Continuity of Care Certification (A-CCC), or other Case Management certification desirable.
