Job Summary

At Catholic Health Initiatives, we are committed to providing high-quality care to our patients. As a Mental Health Social Worker, you will play a vital role in our team, working closely with patients, families, and healthcare professionals to provide comprehensive care.

Responsibilities
  • Conduct and document psychosocial assessments of patients
  • Share responsibility in identifying appropriate decision makers if the patient is without capacity, educating all patient's rights and encouraging participation in decision-making and care
  • Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs
  • Identify mental health needs of patients and provide counseling and referral as indicated
  • Collaborate with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community
  • Serve as a resource to patient, family, and healthcare team regarding emotional and behavioral issues and refer to community services available to adolescents, adults, and elders
  • Remain current of Medicare and Medicaid laws and guidelines, Medicare conditions of participation, managed care contracts, and other legal and regulatory requirements as they relate to healthcare social work services
Qualifications
  • Bachelor's or Master's degree from a Council of Social Work Education accredited School of Social Work
  • State of Nebraska certification/licensure in Social Work required
Pay Range

$29.20/hour
