Job Summary

We are seeking a skilled Social Worker-Outpatient Care Coordinator to join our multidisciplinary team in the Outpatient CHI Health Clinics. As a key member of our team, you will play a vital role in assessing and providing support and intervention for the social and emotional needs of patients and their families to promote improved health, continuity of care and services in the outpatient setting.

Responsibilities
  • Identify member populations needing care coordination and social work intervention.
  • Act as a liaison with patients and families to physicians, clinical staff, community resources and others.
  • Conduct psychosocial assessments and act as a patient's advocate, responding to and working with patients to address needs and resolve concerns.
  • Provide Health Promotion activities and education for patients.
  • Collaborate effectively with healthcare professionals to eliminate fragmentation, duplication or gaps in healthcare.
  • Optimize patient and family independence and self-determination by providing education and health coaching, and assisting with improving problem-solving and coping skills as needed.
  • Work with patients, families, and caregivers to address barriers affecting health, including economic needs, behavioral issues, services in home, disability, out of home placement, etc.
  • Identify and link families to community resources.
Qualifications

Required Licensure and Certifications:

Current/Active Certification to practice Social Work in the State of Nebraska is required (CSW).

Required Minimum Knowledge, Skills, Abilities and Training:

Minimum 3 years clinical experience as a Social Worker required. Minimum 5 years managed care or equivalent care management experience preferred. Understanding of the roles of the healthcare team. Deep understanding of the psychosocial aspects of illness and health. Compassionate demeanor with patients of all ages and conditions. Knowledge of community facilities, resources and entitlement programs. Excellent short-term counseling and crisis intervention skills. Experience working with Medicare and Medicaid populations, and related social/economic issues preferred.

Skills:

Strong organizational (time management) skills, strong interpersonal skills, and the ability to handle multiple priorities. Knowledge of and practical use of good business English, spelling, arithmetic, practices and the ability to communicate effectively using written and verbal skills. Proficient in email communications and internet usage along with basic use of Microsoft Excel and Word. Knowledge of information technology to evaluate care effectiveness (care process, outcomes and cost) for individual users of healthcare and patient populations.

Exhibits professionalism and ability to build rapport and maintain relationships with providers.

Pay Range

$29.20/hour
