About the Role

SSM Health Care System is seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team in a primary care setting. As a key member of our team, you will provide high-quality patient care, emphasizing health promotion, disease prevention, and patient education.

Key Responsibilities
  • Conduct Comprehensive Health Assessments
    • Conduct thorough patient histories, physical exams, and diagnostic tests to identify health needs and develop individualized care plans.
  • Develop and Implement Care Plans
    • Collaborate with patients and families to develop and implement care plans that address their unique health needs and goals.
  • Diagnose and Manage Acute and Chronic Illnesses
    • Diagnose and manage acute and chronic illnesses, including prescribing medications and other treatments as needed.
  • Provide Preventive Care
    • Provide routine check-ups, immunizations, and health education to promote healthy lifestyles and prevent disease.
  • Collaborate with Healthcare Professionals
    • Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care and optimal patient outcomes.
Qualifications
  • Education and Certification
    • Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
    • Certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Experience and Skills
    • Minimum of 2 years of experience in a primary care setting.
    • Excellent clinical, diagnostic, and therapeutic skills.
    • Strong communication and interpersonal skills.
    • Ability to work collaboratively in a team-oriented environment.
Work Environment
  • Full-time Position
    • Full-time position with regular office hours.
  • Occasional Evening or Weekend Shifts
    • May require occasional evening or weekend shifts based on patient needs.
