Job Summary

SSM Health Care System is seeking a dedicated and compassionate Family Nurse Practitioner to join our healthcare team. As a key member of our primary care team, you will provide high-quality patient care services to individuals of all ages, focusing on health promotion, disease prevention, and patient education.

Key Responsibilities:
  • Conduct Comprehensive Health Assessments: Conduct thorough health assessments, including patient histories, physical exams, and diagnostic tests, to identify and address patient needs.
  • Develop and Implement Care Plans: Develop and implement individualized care plans based on patient assessments, ensuring that patients receive the best possible care.
  • Diagnose and Manage Acute and Chronic Illnesses: Diagnose and manage acute and chronic illnesses, providing patients with the necessary treatment and support.
  • Prescribe Medications and Treatments: Prescribe medications and other treatments in accordance with state regulations and practice guidelines.
  • Provide Preventive Care: Provide routine check-ups, immunizations, and health education to promote healthy lifestyles and prevent illnesses.
  • Collaborate with Healthcare Professionals: Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care and provide comprehensive patient care.
  • Educate Patients and Families: Educate patients and families on managing medical conditions and promoting healthy lifestyles.
  • Maintain Accurate Patient Records: Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.
  • Participate in Quality Improvement Initiatives: Participate in quality improvement initiatives and professional development activities to enhance patient care and outcomes.
Qualifications:
  • Master's Degree in Nursing: Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Licensure and Certification: Current licensure as a Nurse Practitioner in the state of Missouri and certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
  • Experience: Minimum of 2 years of experience in a primary care setting preferred.
  • Clinical, Diagnostic, and Therapeutic Skills: Excellent clinical, diagnostic, and therapeutic skills.
  • Communication and Interpersonal Skills: Strong communication and interpersonal skills.
  • Teamwork: Ability to work collaboratively in a team-oriented environment.
  • EHR Proficiency: Proficiency with electronic health records (EHR) systems.
Work Environment:
  • Full-time Position: Full-time position with regular office hours.
  • Occasional Evening or Weekend Shifts: May require occasional evening or weekend shifts based on patient needs.
