Job Title: Clinical Support Specialist

Job Summary: We are seeking a highly skilled Clinical Support Specialist to join our team at Independent Living Association. The successful candidate will provide clinical support to individuals with developmental disabilities, ensuring their health and well-being needs are met.

Key Responsibilities:

  • Assist the facility nurse and psychologist in providing medical services to individuals.
  • Coordinate medical appointments, communicate results, and schedule follow-up appointments.
  • Serve as a liaison between the nurse, psychologist, and residence manager.
  • Coordinate scheduling of follow-up appointments and ensure follow-up visits are carried out under the direction of the nurse.
  • Prepare clinic consults and other required paperwork for appointments in the absence of the nurse.
  • Maintain sufficient medication supply and equipment in the absence of the nurse.
  • Report to the facility nurse, psychologist, and manager regarding appointments and follow-up information.
  • Communicate with day programs regarding individual's medical appointments and changes in medical condition.
  • Inform management if any additional supports are needed for the individual's medical appointments.
  • Accurately use Precision Care software to maintain and track individual's medical information.

Requirements:

  • Valid Driver's License
  • HS Diploma/GED
  • Proficient reading and writing skills
  • Effective communication skills
  • Knowledge of basic medical terminology
  • Good organizational and time management skills
  • Basic computer skills
  • Valid AMAP certification

Working Conditions: The successful candidate will work in a fast-paced environment, providing clinical support to individuals with developmental disabilities. The individual must be able to lift and transfer up to 25 pounds and may be assigned to work with individuals in wheelchairs, walkers, and operate Hoyer lifts if needed.
