Key Responsibilities:
As a Practical Nurse Specialist, you will:
- Deliver in-home direct patient care in accordance with the prescribed care plan while closely monitoring patient progress and recovery.
- Assess and document patient conditions, reporting any changes as necessary (including vital signs, blood/stool/urine, glucose levels, etc.).
- Administer medications, insulin, and IV fluids: ensuring thorough documentation.
- Inspect and manage wounds, change dressings, and assist with personal hygiene needs.
- Evaluate patients for potential injuries, infections, bedsores, etc., and provide appropriate treatments.
- Collaborate with healthcare providers, aides, and allied health professionals to achieve exceptional patient outcomes.
Qualifications:
To be considered for the Practical Nurse Specialist position, candidates must meet the following criteria:
- Education: Graduate of a practical (vocational) nursing program and possess a current LPN/LVN license in the relevant state(s) with at least 1200 hours of experience or completion of the Office's preceptor program.
- Current CPR/AED/BLS/First Aid certification.
- Reliable transportation to and from care sites.
- Experience with tracheostomy and/or ventilator care is preferred but not mandatory.
