Job Summary
We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and diagnosis and treatment planning for our patients. You will work collaboratively with our team of healthcare professionals to provide high-quality care and improve patient outcomes.
Responsibilities
- Conduct adult and geriatric assessments, medical history reviews, and diagnosis and treatment planning
- Work collaboratively with our team of healthcare professionals to provide high-quality care and improve patient outcomes
- Provide education and support to patients and their families
- Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination
Requirements
- Master's degree in nursing or related field
- Current RN and NP licensure in state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent verbal and written communication skills
