Care Coordination Specialist

The role of the Care Coordination Specialist at Vi Living requires a skilled and compassionate Registered Nurse to oversee the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI). This position involves collaborating with healthcare professionals to manage assessment processes, ensuring optimal reimbursement while providing exceptional floor nursing care.

Key Responsibilities:
  • Collaborate with senior leadership to complete MDS, RAI, and associated processes
  • Manage resident assessment process and tracking of Medicare/Managed Care/Medicaid case mix documents
  • Assist with coordination of resident care planning processes under oversight from the Director of Nursing or Clinical Risk Manager
Requirements:
  • Graduation from a Registered Nursing program is mandatory
  • A minimum of 1 year experience in MDS/Clinical Reimbursement Nurse roles is preferred
  • Current State Licensure as a Registered Nurse in the practicing state is required
  • CPR, AED, and First Aid certifications are necessary
