Job Summary

We are seeking a skilled Healthcare Coordination Specialist to join our team at San Francisco Health Plan. In this role, you will play a vital part in ensuring seamless care transitions for our members. If you have experience in customer service, data collection, and coordination, we encourage you to apply.

About the Role
  • You will work closely with our multi-disciplinary team to support discharge planning efforts, track utilization management requests, and provide administrative assistance as needed.
  • With your excellent communication skills, you will serve as a liaison between members, providers, and clinics, ensuring that authorization requests are processed efficiently.
  • In this dynamic environment, you will be responsible for maintaining accurate records, tracking census, and collaborating with clinicians to generate correspondence compliance reports.
Requirements
  • A minimum of one year of experience in customer service or a related field.
  • A bachelor's degree in health sciences, social sciences, or a related field is preferred, but not required.
  • Current California Medical Assistant Certification or equivalent experience in healthcare is highly valued.
