Job Summary

We are seeking a highly skilled and dedicated Case Manager/Support Worker to join our team at Centro Las Americas. This is a challenging and rewarding role that requires strong organizational and communication skills, as well as the ability to work effectively with families and community partners.

Key Responsibilities
  • Conduct assessments and develop service plans with clients and their families
  • Provide direct services to clients, including skills building, service connection, and navigation
  • Collaborate with DCF case workers and other stakeholders to ensure effective case management
  • Participate in staff orientation and training, and contribute to the development of program quality
  • Document interactions with clients and family members, and maintain accurate records
Requirements
  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or Social Rehabilitation
  • LSW License required
  • Excellent written and oral communication skills
  • Strong organizational and leadership skills
  • Able to work independently and as part of a team
