Job Description:About the Role

This position plays a critical role in supporting our mission to deliver high-quality social services to those who need them most.

The Case Manager will be responsible for developing and implementing individualized service plans, providing case management services, and collaborating with community partners.

This is an exciting opportunity for a dedicated and compassionate professional to make a real difference in the lives of our clients.

Responsibilities
  • Implement the individual and family case management services program.
  • Collaborate with clinical personnel, businesses, schools, and parents on programmatic partnerships.
  • Develop and facilitate Individual Service Plans (ISPs) for clients.
  • Provide case management services, including assessment, goal-setting, and resource coordination.
  • Maintain accurate records and reports on client progress.
  • Participate in staff meetings and training sessions to enhance skills and knowledge.
Requirements
  • Bachelor's degree in social work, psychology, sociology, or related field.
  • Minimum 2 years' experience working with at-risk youth or vulnerable populations.
  • Certification in a relevant discipline (e.g., certified addictions counselor) may be required.
  • Excellent communication, interpersonal, and organizational skills.
