We are seeking a highly skilled Behavioral Healthcare Coordinator to join our team at Piedmont HealthCare. As a key member of our Behavioral Health department, you will work closely with patients, families, and healthcare teams to provide exceptional care and support.

About the Role

The ideal candidate will have at least 2 years of experience working in a Behavioral Health setting and hold a current Georgia license for Advanced Practice Nursing or a Physician Assistant license. A Master's degree in Nursing or a related field is required.

Key Responsibilities
  1. Assess patient needs and develop individualized care plans to address their unique needs.
  2. Collaborate with interdisciplinary teams to provide coordinated care and support services.
  3. Provide education and resources to patients and families to promote healthy behaviors and outcomes.
  4. Facilitate communication between patients, families, and healthcare teams to ensure seamless care coordination.
  5. Maintain accurate and up-to-date records of patient progress and care activities.
