Key Responsibilities- Provide professional, independent nursing care to patients in the PACE/Elderplace program
- Assess, plan, order, give, delegate, teach, and supervise professional nursing care delivery
- Work collaboratively with the interdisciplinary team to develop and implement individualized care plans
- Participate in ongoing education and training to stay current with best practices and regulatory requirements
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association certification within 30 days of hire
- Relevant certifications applicable to the position, as specified by the Nurse Manager
- Bachelor's Degree in Nursing
- 1 year Home health RN experience
