Job Summary

We are seeking a compassionate and dedicated Medical Social Worker to join our team at Orange Hospice. As a Per-Diem Hospice Medical Social Worker, you will provide crucial support to our patients and their families, utilizing your exceptional interpersonal skills and community resources to deliver high-quality end-of-life care.

Responsibilities
  • Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and financial needs through in-person visits and phone calls.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team to address identified needs and goals.
  • Provide counseling and emotional support to patients and families facing end-of-life issues, grief, and bereavement.
  • Assist patients and families in accessing community resources and support services.
  • Document all assessments, interventions, and interactions on HospiceMD in accordance with organizational and CHAP standards.
  • Collaborate with interdisciplinary team members, including physicians, nurses, chaplains, and volunteers, to ensure comprehensive care delivery.
  • Participate in staff IDG meetings to discuss patient and family care plans.
Requirements
  • Bachelor's or Master's degree in Social Work from an accredited institution. Medical Social Worker (MSW) or Licensed Social Worker (LSW) preferred.
  • Experience working in hospice care.
  • Korean speaking highly preferred.
  • Excellent communication and interpersonal skills.
  • Valid driver's license and reliable transportation for local travel.
  • Willing to travel to Los Angeles, Orange, and Riverside counties.
  • Ability to work independently and as part of a multidisciplinary team.
  • Compassionate and empathetic approach to patient care.
  • Strong organizational and time-management skills.
