We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at Haven Hospice. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and families in their homes.
Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide physical assessments, write and initiate plans of care, and make necessary revisions as patient status and needs change.
- Administer medications and treatments as prescribed by the physician, and counsel patients and families on meeting their nursing and related needs.
- Assist patients with activities of daily living, facilitate their efforts toward self-sufficiency, and provide comfort care.
- Act as Case Manager when assigned, coordinating patient care for assigned caseloads and ensuring continuity of care.
- Communicate with the physician regarding patient needs, report changes in patient condition, and obtain/receive physicians' orders as required.
- Teach patients and families/caregivers self-care techniques, provide medication, diet, and other instructions as ordered by the physician, and recognize opportunities for health counseling.
- Work in concert with the interdisciplinary group, providing and maintaining a safe environment for patients, and ensuring arrangements for equipment and other necessary items and services are available.
- Graduate of an accredited school of Nursing.
- Current Registered Nurse licensure in State and American Heart Association CPR certification required.
- Bachelor's degree preferred, with one (1) year of hospice experience preferred.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
