Job Overview

Project Transition is dedicated to empowering individuals with serious mental illness, co-occurring substance use disorder, and dual diagnoses to live a meaningful life in their community. We are seeking a highly motivated and compassionate Community Support Specialist to join our team.

We offer a competitive salary of $45,000 - $55,000 per annum, commensurate with experience. If you are passionate about making a difference in the lives of others, please consider this exciting opportunity.

About the Role

The successful candidate will be responsible for developing and implementing individualized training and treatment plans for assigned members, participating in crisis management systems, and providing supervision and support in daily living activities. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Implement and document member interventions and progress relative to treatment/recovery plans.
  • Complete Incident Reports following established procedures.
  • Provide supervision for assigned member activities and trips.
  • Train and supervise members in activities of daily living.

Requirements

This position requires at least a High School Diploma and two years' experience in therapeutic communities and/or psychiatric care. A valid driver's license is also essential. We are an equal opportunity employer and welcome applications from diverse candidates.
