Job Overview

At Wraparound Maryland, we are seeking an experienced and skilled Care Coordinator Supervisor to join our team. This is a full-time position offering a competitive salary range of $52,000.00 - $65,000.00 per annum.

About the Role

The successful candidate will provide coaching and support to care coordinators, ensuring they effectively engage with families and utilize the wraparound process. They will also review and approve Plans of Care, evaluate adherence to wraparound principles, and participate in home visits, phone calls, and other support activities between Child and Family Team (CFT) meetings.

Key Responsibilities

  1. Provide coaching and guidance to care coordinators on engagement strategies and the creation of effective Child and Family Teams.
  2. Support care coordinators in identifying community resources that benefit children and families, developing positive relationships with providers, and ensuring access to quality services.
  3. Maintain ongoing awareness of community resources and facilitate collaboration between care coordinators, families, and providers.
  4. Participate in CFT meetings as needed and contribute to the development of Plans of Care.
  5. Review and approve Plans of Care for adherence to wraparound principles and ensure timely completion of tasks.
  6. Collaborate with the Clinical Program Supervisor on joint supervision and training of newly hired care coordinators.
  7. Represent Wraparound Maryland at community meetings, promoting the organization's mission and values.
  8. Maintain accurate documentation and data collection, supporting the program's implementation and evaluation strategy.
  9. Pursue professional growth opportunities, staying current with industry developments and best practices.

Qualifications and Requirements

  • A Master's Degree in a human services field or equivalent experience.
  • License in Good Standing (LGPC or LMSW) required.
  • Two years of experience in human services, including one year in community-based service provision and one year working with children, youth, and families.
  • Excellent interpersonal skills, ability to work effectively with diverse constituencies, and strong verbal and written communication skills.
  • Able to analyze complex information, solve problems, and manage multiple priorities simultaneously.
  • Valid driver's license and current CPR and First Aid Certification necessary.
