Responsibilities:

  • Develop and implement strategies to improve the quality of care provided by care coordinators.
  • Collaborate with other departments and teams to ensure seamless communication and coordination.
  • Participate in joint supervision with Clinical Program Supervisor as necessary.

Requirements:

  • A Master's Degree in a human services field or equivalent experience.
  • Licensed Graduate Professional Counselor (LGPC) or Licensed Master Social Worker (LMSW).
  • Two years of experience in a human services position, including at least one year of experience working in community-based service provision and at least one year of experience working with children, youth, and families.
  • An understanding of child and adolescent development.
  • Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.
  • Strong interpersonal skills and ability to work effectively with a wide range of constituencies in a diverse community.
  • Effective verbal and written communication skills.
  • Ability to develop and deliver a case presentation.
  • Strong organizational skills and detail-oriented.
  • Ability to analyze complex information and define and solve problems.
  • Ability to work effectively in a team environment.
  • Manage time and work effectively with minimal supervision.
  • Effectively manage multiple priorities simultaneously.
