Job SummaryThe Registered Nurse in Home and Community Based Services plays a vital role in delivering high-quality clinical services to patients in their homes. As a member of our care team, you will work collaboratively with other healthcare professionals to develop and implement individualized plans of care that meet the unique needs of each patient.
Responsibilities- Provide skilled nursing services to patients in their homes, ensuring the highest level of quality and safety.
- Conduct comprehensive assessments of patients' medical conditions, identifying areas for improvement and developing strategies to address these needs.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless coordination of care.
- Develop and implement individualized plans of care that meet the unique needs of each patient, incorporating input from patients, family members, and caregivers as needed.
- Monitor patients' progress, making adjustments to care plans as necessary to ensure optimal outcomes.
To be successful in this role, you will need:
- A current RN licensure in the state of practice.
- A minimum of one year experience as an RN, preferably in home health or a related field.
- Current CPR certification.
- Access to a dependable vehicle or public transportation.
