Caregiver for Medically Fragile Children and Adults

Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team.
Job DescriptionThe LPN/LVN will provide high-quality patient care in the comfort of their own homes, under the supervision of a Registered Nurse. This role requires strong clinical assessment and documentation skills, as well as excellent communication and problem-solving abilities.
Key Responsibilities:
- Deliver and coordinate quality patient care in compliance with physician orders
- Continuously observe and assess patient condition and care needs
- Document all activities, assessments, nursing actions, responses, and coordination of care
- Participate in updating the nursing care plan
- Implement appropriate nursing action based on assessment and achieve expected outcomes
Requirements:
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
- Six months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience
