Job Summary:

Aveanna Healthcare seeks a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to provide high-quality patient care in the comfort of their own homes. Responsibilities:
  • Provide and document skilled nursing care in accordance with physician orders and developed care plans
  • Conduct continuous observations and assessments of patient condition and care needs
  • Report changes in condition to supervisors and physicians as necessary
  • Implement and update nursing care plans to ensure optimal patient outcomes
  • Educate patients and families on disease management, self-care techniques, and prevention strategies
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies
Preferred Qualifications:
  • Six months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
