Position:
Clinical Program Coordinator

Compensation:
Starting at $70000 / annually, contingent upon education, experience, and licensure.

Key Responsibilities:


The Clinical Program Coordinator will support the Program Director in the overall management of the program, which includes scheduling, shift coverage, training, on-call responsibilities, and community representation as needed.

Accountable for the planning, development, and assessment of program effectiveness.

Responsible for providing direct oversight of individuals residing in respite care and supervising all support staff.

Conducts chart audits to ensure compliance with agency and licensing standards in electronic health records.

Manages discharge planning and follow-up care.

Assists in staff meetings, documentation of meeting minutes, and maintenance of training resources.

Shares 24-hour on-call duties with the Program Director.

Utilizes SCM, CPR, and First Aid techniques as necessary.

Ensures completion of comprehensive intake assessments and ongoing nursing evaluations by qualified staff; individual, group, and family counseling; Crisis Intervention Counseling; and informing residents of their rights and procedures regarding potential removal or discharge from the residence.

Provides clinical support for staff around the clock.

Qualifications:
A minimum of three years of experience in psychiatric nursing is required.

A valid license in New Jersey is mandatory.

Strong verbal and written communication skills are essential for interacting with multidisciplinary teams.

Must possess a valid driver's license, and if driving is required, be capable of operating an agency vehicle.

Physical abilities to walk, run, lift, push, pull, and bend are necessary.

Cognitive, verbal, and manual dexterity skills are required.

Hearing and visual acuity are necessary; corrective devices are permissible.

Must be able to work under tight deadlines while remaining adaptable to special requests and projects.

Ability to manage stressful situations in a professional manner.

Crisis Diversion Homes

This program offers support to adults who have recently faced a crisis, working closely with the 988-crisis continuum.

The initiative provides psychiatric stabilization and case management services in a residential environment to prevent hospital admissions and emergency department visits.

The anticipated length of stay is less than 30 days.
