The NYC Health Hospitals is seeking an experienced Ambulatory Care Pharmacist to join its team. This role will play a vital part in providing exceptional patient care across the health system.
About the Role:
This position will work closely with the Office of Ambulatory Care and collaborating physicians to support disease state management, identifying ways to optimize medication therapy and ensuring an optimal patient experience.
Key Responsibilities:
- Conduct thorough interviews with patients to gather relevant data, including medical history, medication history, and immunization history.
- Screens for drug allergy and drug interactions, noting contraindicated drugs or classes of drugs.
- Assesses medication regimens and makes modifications to achieve desired therapeutic outcomes.
- Performs routine patient monitoring functions, collecting and reviewing patient history, and ordering necessary laboratory and physical assessments.
Requirements:
- A valid New York State license and current registration to practice pharmacy.
- Certification to practice Collaborative Drug Therapy Management (CDTM) by the New York State Education Department (NYSED).
Compensation:
$120,000 - $150,000 per year.
