Job Description

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team As an LPN, you will work under the supervision of a provider or registered nurse to assess, plan, implement, and evaluate patient care. You will perform nursing process while providing direct patient care in a safe and effective manner that promotes quality patient outcomes.

Key Responsibilities:

  • Assists licensed professional personnel in performing nursing process across the continuum to deliver age and developmentally appropriate patient-family-centered care.
  • Utilizes clinical decision support tools and resources to recognize issues and prioritize care.
  • Provides patient and family education appropriate for identified learning needs.
  • Communicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs.

Requirements:

  • No minimum experience required.
  • High school diploma or equivalent.
  • Completion of an accredited Licensed Practical Nurse (LPN) school/program.
  • Licensed Practical Nurse (LPN) certification.

Preferred Qualifications:

  • 1 year of patient care experience preferred.
