We're seeking a skilled Nurse Practitioner or Physician Assistant to join our Landmark product team. As a member of our interdisciplinary team, you'll work closely with patients, primary care physicians, and other healthcare professionals to develop and implement personalized care plans that improve health outcomes and quality of life.

Key Responsibilities:
  1. Conduct comprehensive assessments and develop individualized care plans for patients with complex, chronic conditions
  2. Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal health outcomes
  3. Prescribe medications and interventions to manage chronic conditions and prevent unnecessary hospitalizations
  4. Provide education and support to patients and families on disease management, wellness, and self-care
  5. Participate in ongoing quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Completion of a Nurse Practitioner or Physician Assistant program accredited by the Commission on Collegiate Nursing Education (CCNE) or the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
  • Current state RN/NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
  • Access to reliable transportation
